First and foremost, our Employee Screening Form is a quick and easy way to gather pertinent health information from your employees at the start of each workday. These forms ask relevant questions about potential symptoms and exposure to COVID-19, employing decision logic to suggest whether individuals should stay home until they are tested.
If someone does test positive, that’s when our Persons Under Investigation Form comes in handy. With this one, businesses can keep a solid record of employees’ points of contact (both colleagues and customers), tracking those who were potentially exposed. These people can be alerted and encouraged to get tested as well, without having to reveal the infected party’s identity.
Contactless delivery is something we’ve all become very familiar with as of late. To help you keep a record of these deliveries — since folks aren’t signing for packages anymore — we came up with yet another form.
